Introduction
This Privacy Policy (“Policy”) describes the data protection practices of Custom Health Centers Inc. dba ELEO (“CHC”, “ELEO,” “we,” “us,” or “our”). This Policy applies to all websites, mobile applications, and other services we provide (collectively, the “Services”). This Policy is incorporated into our Terms of Service; undefined capitalized terms have the meanings given in those Terms.
ELEO is headquartered in Michigan, and our Services are available to users in all 50 states.

1. The Information We Collect and the Sources of Such Information
We collect information about you in order to provide and improve the Services. If you choose not to provide required information, or if you ask us to delete it, you may be unable to access or use portions of the Services.
1.1 Information You Provide to Us
When you register for an account or use the Services, you may directly provide:
Account & Order Details: Name, address, email, phone number, date of birth, account credentials, billing and payment information (e.g., credit card data via our payment processor), and order history.
Health & Wellness Data: Details about medical conditions, treatments, prescriptions, lab results, medical images, lifestyle preferences, and other health-related information.
Communications & Feedback: Questions, support requests, survey responses, contest entries, and any content you upload or post.
Demographics & Contacts: Gender, age, race/ethnicity, marital status, emergency contact information, and similar.
Employment Applications: Resumes and application materials if you apply to work with us.
1.2 Information Collected Through Automated Means
When you interact with the Services, we and our third-party service providers automatically gather:
Usage & Device Data: IP address, browser type and language, operating system, device identifiers, referring and exit URLs, pages viewed, clickstreams, date/time stamps, time on page, domain names, download links, and error logs.
Location Data: General location inferred from IP address or Wi-Fi/cell tower signals (city/state or postal code). With your permission, we may collect precise GPS data to provide location-based content and promotions. You can disable GPS collection in your device settings at any time.
Cookies & Tracking Technologies: We use cookies, web server logs, SDKs, pixels, and similar tools to:
Recognize and authenticate your device
Store preferences and settings
Analyze site use and performance
Deliver personalized content and advertising
Enhance security and administrative functions

1.3 Information from Social Media and Content Platforms
If you choose to connect your social media account (e.g., Facebook, Instagram, Twitter) or follow us on those platforms, we may receive your name, email, and any public comments you post about ELEO. Promotional entries or form submissions via social media will also share data with us as permitted by your social platform settings.
1.4 Information from Other Sources
We may receive additional information about you from our partners—such as physicians, pharmacies, marketing partners, analytics providers, and advertising networks—to enhance and customize your experience with our Services.
2. How We Use Your Information
Unless you opt out via our Cookie Consent tool, we may use the information we collect for our business purposes to:
Deliver, improve, and manage the Services. Facilitate telehealth and pharmacy services by coordinating with physicians and other healthcare providers licensed in Michigan (and other states where we operate), and ensure they have the support needed for operations.
Conduct internal research. Analyze aggregated, de-identified data to evaluate and enhance our Services and better understand our user base. Any published results will not identify individual users.
Communicate with you. Send transactional messages about your account or Service use, and deliver communications from your providers or pharmacies to meet your healthcare needs.
Send marketing and promotional materials. In compliance with applicable laws, we may contact you by email, postal mail, or phone about surveys, special offers, events, and our products and Services (including those of our affiliates and partners).
Provide support. Offer technical assistance, troubleshoot issues, and respond to your customer-service inquiries.
Verify identity and administer your account. Process payments, fulfill orders, and confirm the authenticity of your government-issued ID or signature when required.
Optimize content and features. Present information in the most effective format for your device, enable interactive Service features when you choose to use them, and maintain the security and integrity of our Services.
Personalize advertising and content. Deliver relevant ads and measure their effectiveness, as well as provide content tailored to your interests.
Understand interests and needs. Perform analysis and research to better serve you and improve our offerings.
Comply with legal and regulatory obligations. Follow applicable laws and act to prevent prohibited or illegal activities in furtherance of our legitimate interests or those of others.
Enforce and protect our rights. Establish, exercise, or defend our legal rights and safeguards against fraud, abuse, or unlawful activity.
Other disclosed purposes. Use your information for any additional purposes we disclose at the time of collection or pursuant to your consent.
Health Information: Any health data we collect is subject to additional protections under state law. We will use, process, and disclose such data—and instruct our service providers to do the same—in compliance with the laws of your state of residence.
Combined Information
We may merge the information collected through the Services with data we receive from other sources, both online and offline, and use or share this combined information in accordance with this Policy and any required consents.
Aggregate & De-Identified Data
We may aggregate and/or de-identify information so it cannot be linked to you or your device (“Aggregate/De-Identified Data”). We may use and share this Aggregate/De-Identified Data for any purpose—including research, marketing, or distribution to advertisers, partners, and sponsors—without restriction.
3. Online Analytics and Advertising
3.1 Online Analytics
To understand and improve our Services, we and our service providers use third-party web analytics tools—such as Google Analytics (including Google Signals, User-ID, and related features) and MixPanel—that collect and analyze usage information via cookies, SDKs, pixels, and similar technologies. These tools help us:
Audit and report on Service usage;
Prevent fraud and enhance security;
Recognize unique users across devices to tailor features;
Provide you with personalized functionality.
If you have a Google account with personalized advertising enabled, Google Signals may link your activity across devices. To opt out of Google Analytics collection (including cross-device tracking), you can install the Google Analytics Opt-Out Browser Add-On. To disable Google Signals on Android devices, open Settings → Google → Ads and enable Opt out of Ads Personalization. On iOS, you can limit ad tracking in Settings → Privacy → Apple Advertising.
For email communications, we may use tracking pixels (clear GIFs) to see when you open our messages or click links. This helps us gauge the effectiveness of our outreach.
3.2 Online Advertising
We partner with third-party ad networks and servers (e.g., Facebook Ads, Google Ads, TikTok, Criteo) that use cookies, pixels, and similar tools to deliver relevant ads for ELEO products—both on JoinELEO.com and across other sites and apps you visit. Ads may be based on page content, your searches, demographics, and other information we collect.
We may share hashed or de-identified customer data (such as email addresses) with these networks so they can match you to their identifiers and serve you more relevant ads. We do not control the placement of cookies or tracking by non-affiliated ad networks.
To opt out of targeted display advertising:
Google Display Ads: Visit your Google Ads Settings.
Facebook Ads: Manage your Facebook Ad Preferences.
TikTok Ads: Adjust settings in the TikTok app under Settings → Privacy → Personalization & data.
Please note that opting out will not stop all advertising—it will only disable personalized ads based on your browsing history.
3.3 Mobile Advertising
Within third-party mobile apps, you may receive tailored in-app ads delivered by our advertising partners. Each mobile platform (iOS, Android, Windows) provides settings to limit personalized advertising:
iOS: Settings → Privacy → Apple Advertising → Turn off Personalized Ads.
Android: Settings → Google → Ads → Enable Opt out of Ads Personalization.
Windows Mobile: Settings → Privacy → Advertising ID → Turn off.
Because these controls are managed by the device OS and may change over time, please refer to your device’s support documentation for the latest instructions.
4. How We Share and Disclose Your Information
With your consent where required, ELEO may share your information for our business purposes in the following ways:
Affiliates & Subsidiaries. We may share data across ELEO and our corporate affiliates to deliver and improve our Services, ensure a consistent experience, and enhance our offerings.
Health Care Providers & Pharmacies. We share your information with third-party physicians, clinics, and pharmacies to:
Schedule and fulfill telehealth appointments and pharmacy orders;
Transmit messages you send via the ELEO platform; and
Support treatment, payment, and healthcare operations (e.g., prior authorizations, billing).
Service Providers. We engage trusted vendors to perform tasks on our behalf—such as billing, payment processing, data hosting, IT and security, customer support, analytics, marketing, shipping, fraud prevention, and legal compliance. These providers are contractually prohibited from using your information for any other purpose.
Advertising Networks (unless you opt out). We share de-identified or hashed identifiers (e.g., email hashes, device IDs) with ad partners (e.g., Facebook, Google Ads, TikTok) so they can serve you relevant ELEO ads across the web and apps. See Section 3 above for details on opting out of personalized advertising.
Joint Service Providers. When ELEO co-offers a product or service with another company, that partner may collect and use your data under its own privacy policy.
Protection of ELEO and Others. We may access, retain, and disclose your information if we believe, in good faith, that it is necessary to:
Comply with subpoenas, court orders, or other legal processes;
Enforce our Terms of Service or this Privacy Policy, including investigating potential violations;
Respond to claims that content you provide infringes third-party rights;
Address your customer-service requests; or
Protect the safety, rights, or property of ELEO, our users, or the public—such as sharing information with fraud-prevention organizations.
Business Transfers. If ELEO merges, is acquired, or sells all or part of its assets, your information may be among the transferred assets. We will honor any privacy commitments made to you and comply with applicable restrictions.
Public Forums & User Interactions. Any information you post publicly (e.g., product reviews, community forums) may be visible to others and used by ELEO for any lawful purpose, including testimonials or marketing materials. We’re not responsible for how third parties use that information.
Testimonials & Reviews. We may invite you to submit reviews or testimonials. By providing such content, you grant ELEO the right to use your name, likeness, and testimonial in marketing materials and on our sites.
Consent. We may share your information in any other ways you expressly authorize.
Aggregate & De-Identified Data. We reserve the right to aggregate or de-identify personal data so that it can no longer be linked to you. We may use and share such data for research, marketing, or other purposes without restriction.

5. Your Choices
Update or Delete Your Data
You may request that we correct, update, or delete your personal information by emailing us at info@joineleo.com. We will use commercially reasonable efforts to honor your request in accordance with applicable law.
Opt-Out of Marketing Communications
Email: Click the “Unsubscribe” link in any marketing email, or send an email to info@joineleo.com with your updated communication preferences.
SMS: Follow the “STOP” instructions contained in any promotional text, or email info@joineleo.com.
You may continue to receive non-marketing, transactional, or service-related messages even after opting out of marketing messages.
Disable Cookies & Similar Technologies
You can manage your cookie preferences at any time via our Cookie Consent banner (link in the site footer). You may also disable cookies through your browser settings—see your browser’s “Help” section for instructions. To refuse or delete Local Shared Objects (Flash cookies), visit Adobe’s Flash Player Settings. Note that disabling these technologies may limit certain features of the Services.
Do Not Track Signals
Some browsers let you send “Do Not Track” signals. Our Services do not currently respond to these signals. For more on “Do Not Track,” visit allaboutdnt.com.
6. Your Rights
Depending on your state’s laws, you may have the right to:
Opt out of direct marketing: Prevent processing of your data for marketing or profiling.
Access & Portability: Request a copy of your information in a usable format.
Rectification: Update or correct inaccurate or incomplete data.
Deletion: Ask us to delete your personal information (sometimes called the “right to be forgotten”).
Restriction: Request that we limit how we process or share your data.
Data Transfer: Have your information transmitted to another service provider.
Revoke Consent: Withdraw consent you’ve given for any data processing.
To exercise any of these rights, email info@joineleo.com with your name, address (including your state of residence), and email address. We may ask for additional information to verify your identity. We will respond in accordance with the timelines required by your state’s law.
If we refuse your request, you may appeal by emailing info@joineleo.com with the subject line “Data Rights APPEAL,” including your original request and grounds for appeal. We will handle appeals under applicable law. You may also file a complaint with your state’s Attorney General; find contact details at Find My AG.
Shine the Light (California Residents)
California residents may request, once per year, disclosure of certain categories of personal information shared with third parties for their direct marketing. To make a Shine the Light request, email info@joineleo.com with “California Shine the Light Request” in the subject.
7. How We Protect Your Information
ELEO employs a range of administrative, technical, and physical safeguards designed to protect your personal data against accidental loss, unauthorized access, disclosure, alteration, or destruction. These measures include encryption, secure data centers, access controls, and regular security assessments. However, no system is perfectly secure. You acknowledge that any information you transmit to or through our Services is at your own risk. Where we provide you with a password or other credentials, you are responsible for keeping them confidential and for all activity under your account. Please do not share your password, and be mindful that any information you post in public areas of our Services can be viewed by other users.
8. Children
ELEO’s Services are not intended for use by individuals under 18. We do not knowingly collect personal information directly from children under 13. If we learn that we have inadvertently gathered personal data from a child under 13, we will promptly delete that information. If you believe a child under 13 has provided us with personal data, please contact us immediately at info@joineleo.com so we can take corrective action.
9. Retention of Your Information
We retain your personal data only as long as necessary to fulfill the purposes described in this Privacy Policy, to support our business operations, and to comply with legal obligations. When determining retention periods, we consider factors such as the nature and sensitivity of the information, the risk of harm from unauthorized use or disclosure, and any applicable statutory requirements. Once data is no longer needed, we securely delete or anonymize it.
10. Revisions to Our Privacy Policy
ELEO reserves the right to update this Privacy Policy at any time to reflect changes in law, our practices, or new features of our Services. The “Effective Date” at the top indicates when the Policy was last revised. We encourage you to review the Policy periodically. If we make material changes that affect your rights or how we use your data, we will provide notice as required by law (for example, via email or a site banner). Continued use of the Services after such notice constitutes your acceptance of the updated Policy.
